Description

Event Objectives: 
 1. Close the following gaps:

·         AWE (patients will be encouraged to make an Annual Wellness Exam appointment if they have not already had one this year)
·         DRE (on site screening in the Mobile Unit)
·         A1C (in home kit distribution, or can be done in your office if it is your patient )
·         NEPH-Kidney (in-home kit distribution, or can be done in your office if it is your patient)
·         COL (Patients will be encouraged to make a colonoscopy appointment in office, but if hesitant can receive our in-home kit distribution)
·         OMW (on site screening in the Mobile Unit)
·         BCS (patients will be encouraged to make an appointment for a breast cancer screening)
 
 

2. Support MyHealthpath Rewards activation (our incentive program for preventative care)

3. SilverSneakers engagement (a Silver Sneakers representative will be onsite to encourage members to use that free membership they have at area fitness locations)
