Description
Event Objectives:
1. Close the following gaps:
· AWE (patients will be encouraged to make an Annual Wellness Exam appointment if they have not already had one this year)
· DRE (on site screening in the Mobile Unit)
· A1C (in home kit distribution, or can be done in your office if it is your patient )
· NEPH-Kidney (in-home kit distribution, or can be done in your office if it is your patient)
· COL (Patients will be encouraged to make a colonoscopy appointment in office, but if hesitant can receive our in-home kit distribution)
· OMW (on site screening in the Mobile Unit)
· BCS (patients will be encouraged to make an appointment for a breast cancer screening)
- Support MyHealthpath Rewards activation (our incentive program for preventative care)
- SilverSneakers engagement (a Silver Sneakers representative will be onsite to encourage members to use that free membership they have at area fitness locations)
